Parry Gib
When a boss successfully parries an attack, it accelerates his swing timer, which in turn makes his next melee attack happen sooner, which does more damage to the tank in the long term. When this causes more damage than the healers were prepared for, a Parry Gib is the result.
The solution, of course, is for melee DPS to stand behind the boss.
